If you are coming down with a group or just want to be prepared ahead of time, below you will find PDF files for the common forms that we use. All participants in our programs, whether diving, snorkeling, or just riding a boat, will be asked to sign a liability and release form. In addition, divers participating in educational programs will have additional forms to complete.

Credit Card Authorization Form	Use this form when authorizing payments for reservations made via E-mail or over the internet.
Diver Registration and Release	To be completed by all divers.
Shark Dive Release	To be completed by all divers joining our Shark Adventure(tm) dives.
Medical History Form	To be completed by all divers joining any of our training programs.
RSTC Medical Exam Guidelines and Authorization	If you need to visit your physician to establish your eligibility for scuba activities, use this form as a guide for the exam and for your doctor to give you the OK.
Snorkel / Rider Liability Release Form	To be completed by all persons who will be traveling on a vessel and not diving.
SUB Registration and Release	To be completed by all persons who will be participating in our SUB Bahamas adventure.
